ABSTRACT

Objective:To investigate the value of a preoperatively MRI-based deep learning (DL) radiomics machine learning model to distinguish low-grade and high-grade soft tissue sarcomas (STS).Methods:From November 2007 to May 2019, 151 patients with STS confirmed by pathology in the Affiliated Hospital of Qingdao University were enrolled as training sets, and 131 patients in the Affiliated Hospital of Shandong First Medical University and the Third Hospital of Hebei Medical University were enrolled as external validation sets. According to the French Federation Nationale des Centres de Lutte Contre le Cancer classification (FNCLCC) system, 161 patients with FNCLCC grades Ⅰ and Ⅱ were defined as low-grade and 121 patients with grade Ⅲ were defined as high-grade. The hand-crafted radiomic (HCR) and DL radiomic features of the lesions were extracted respectively. Based on HCR features, DL features, and HCR-DL combined features, respectively, three machine-learning models were established by decision tree, logistic regression, and support vector machine (SVM) classifiers. The area under the receiver operating characteristic curve (AUC) was used to evaluate the performance of each machine learning model and choose the best one. The univariate and multivariate logistic regression were used to establish a clinical-imaging factors model based on demographics and MRI findings. The nomogram was established by combining the optimal radiomics model and the clinical-imaging model. The AUC was used to evaluate the performance of each model and the DeLong test was used for comparison of AUC between every two models. The Kaplan-Meier survival curve and log-rank test were used to evaluate the performance of the optimal machine learning model in the risk stratification of progression free survival (PFS) in STS patients.Results:The SVM radiomics model based on HCR-DL combined features had the optimal predicting power with AUC values of 0.931(95%CI 0.889-0.973) in the training set and 0.951 (95%CI 0.904-0.997) in the validation set. The AUC values of the clinical-imaging model were 0.795 (95%CI 0.724-0.867) and 0.615 (95%CI 0.510-0.720), and of the nomogram was 0.875 (95%CI 0.818-0.932) and 0.786 (95%CI 0.701-0.872) in the training and validation sets, respectively. In validation set, the performance of SVM radiomics model was better than those of the nomogram and clinical-imaging models ( Z=3.16, 6.07; P=0.002,<0.001). Using the optimal radiomics model, there was statistically significant in PFS between the high and low risk groups of STS patients (training sets: χ2=43.50, P<0.001; validation sets: χ2=70.50, P<0.001). Conclusion:Preoperative MRI-based DL radiomics machine learning model has accurate prediction performance in differentiating the histopathological grading of STS. The SVM radiomics model based on HCR-DL combined features has the optimal predicting power and was expected to undergo risk stratification of prognosis in STS patients.

ABSTRACT

Objective To cross-cultural adapt and test the reliability and validity of Humanistic Relationship Experience Scale (HRES) made by Boscart. Methods Cross-culture Adaptation was carried out by following American Academy of Orthopaedic Surgeons Evidence Based Medicine Committee (AAOS). A convenience sampling was conducted among 180 patients with chronic disease, and then assessed the reliability and validity of HRES. The internal consistency coefficient and the test-retest were used to assess the reliability of HRES, and the content validity index (CVI) and construct validity were used to assess the validity of HRES. Results In order to understand the purpose and content of the scale easily, the Chinese version title of HRES was revised to"Nursing Humanistic Care Experience Scale-Noninfectious Chronic Disease Patients Version (NHCES-NCDP)" after soliciting experts′ advice. The CVI of the scale was 1;factor analysis got three factors, which could explain 67.897%of the total variance. A 0.965 Cronbachαof total scale was obtained, and for each dimension range was 0.876-0.948;the test-retest reliability was 0.710 for the overall scale. Conclusion NHCES-NCDP is reliable and valid, which can be used to assess the experience of nursing humanistic care for patients with chronic diseases, and to explore nurses′implementation of nursing humanistic care.

ABSTRACT

Methamphetamine (MA) is a highly addictive psychostimulant that has effects on central nervous system.Chronic MA useis linked to serious neurotoxic effects and ultimately cognitive impairments, thus understanding the effect of chronic MA use on cognitive function can improve the treatment of MA abusers and reduce MA relapse. In the present study, we summarized the main results of animal experiments, neuropsychological studies and brain imaging studies on MA abuse and cognitive impairment, which may provide the basis for future research and clinical treatment.

ABSTRACT

BACKGROUND:There are many experimental studies about compression mechanics of femur after hip replacement at home and aboard. Therefore, it is very important to study the torque, torsion angle, load-displacement relationship of femur after hip replacement. Comparing and analyzing the properties of compression and torsional mechanics of traditional-type prosthesis and anatomical-type prosthesis is of important significance for studying hip replacement and the stability of artificial prosthesis. OBJECTIVE:To compare and analyze the stability of traditional-type prosthesis and anatomical-type prosthesis by stimulating femoral axial compression and torsion tests after hip replacement, so as to provide biomechanical parameters for clinic practice. METHODS: Twelve specimens of left- and/or right-side femur were selected. The femoral necks of 6 left-side femur specimens were retained as the anatomical-type titanium artificial joint prosthesis group, and the femoral necks of the 6 right-side femur specimens were removed as the traditional-type cobalt-chromium-molybdenum artificial joint prosthesis group. The femur specimens in these two groups were respectively placed onto the electronic universal testing machine workbench and were imposed compression stress at the experimental velocity of 5 mm/min. The corresponding displacement values were read under the force of 20, 40, 60, 80 and 100 N. And then, both ends of the femur specimens from these two groups were placed within the chuck of torsion testing machine, and were imposed torque at the experimental velocity of 1(°)/s. The corresponding torsion angle values were read under the torque force of 5, 10, 15 and 20 N?m. RESULTS AND CONCLUSION: Under the external force of 100 N, the displacement was (2.03±0.06) mm in the traditional-type prosthesis group, and (1.83±0.05) mm in the anatomical-type prosthesis group. Under the torque force of 20 N?m, the torsion angle values of traditional-type prosthesis was (21.7±0.7)°, and that of anatomical-type prosthesis was (13.2±0.4)°. The displacement under the external force of 100 N and the torsion angle values under the torque force of 20 N?m in the anatomical-type prosthesis group were al significantly less than those in the traditional-type prosthesis group (P < 0.05). These results suggest that traditional-type and anatomical-type prostheses have different compression and torsion mechanical properties. The anatomical-type femur prosthesis has a better stability than traditional-type femur prosthesis.

ABSTRACT

Objective To study the association between the cerebral infarction and the angiotensin converting enzyme (ACE) gene rs4646994 and rs35397082 polymorphisms in Binhai area ,Tianjin .Methods Gene sequencing and DNA electrophoresis were used for the detection of the ACE gene single nucleotide polymorphisms (SNPs)(rs4646994 and rs35397082) .53 samples from pa‐tients with acute cerebral infarction and 53 samples from healthy volunteers were used in our study .Serum sample were collected from each group and tested by ACE ELISA .Results There were only deletion type of rs35397082 SNP in both of the control and cerebral infarction group .In the control group ,the number of insertion type of rs4646994 was 45(84 .91% ) ,deletion type was 8(15 . 09% ) and in the patients group ,the number of insertion was 47(88 .68% ) and the deletion was 6(11 .32% ) .There was no signifi‐cant difference between the patients group and the healthy donors (P>0 .05) .The concentration of ACE in control group was high‐er than the patients with acute cerebral infraction (P<0 .05) .Conclusion There is no significant association between the ACE gene polymorphisms(rs35397082 and rs4646994) and cerebral infarction in Binhai area ,Tianjin .The different concentration of ACE is not caused by these two SNPs .In this study ,these two SNPs are not the are not the risk factors of the cerebral infarction in Tianjin based on our study .
